Toward Building Defensible Extended Programs: Challenging the Assumptions of Educational Reform

ABSTRACT: Recent education reform proposals eventually will lead to lengthier professional preparation programs for school health educators. Many of the proposals are based on unwarranted assumptions and myths about teacher education as well as an idealized view of undergraduate liberal studies. In addition to using research findings to challenge these assumptions, trends in accreditation by the National Council on Accreditation of Teacher Education and alternative certification are analyzed. The role of experience in clinical accuracy

A randomized controlled trial was performed to study the effect on various teaching techniques on students performing sacroiliac motion palpation tests. This trial assessed the interexaminer reliability of interns in their final year at a chiropractic college, and compared their results prior to and following 1 year of clinical experience. The study also compared the intra- and interexaminer reliability of experienced clinicians. The results were analyzed via the Kappa coefficient. Kappa values for interns ranged from 0.00 to 0.30, with no significant differences noted at the end of 1 year of clinical experience. The interexaminer reliability of experienced clinicians was 0.00 to 0.167, whereas their intraexaminer reliability ranged from 0.15 to 1.00. These results question the role of experience in improving clinical accuracy between examiners performing sacroiliac motion palpation. Results analyzed for intraexaminer agreement were moderate to almost perfect. We conclude that experience does not play a significant role in the diagnostic test analyzed, but rather that clinicians may establish their own criteria by which to determine the standards of a given test.

The Integration of an Educational Program into a Treatment Facility

This paper reviews the integration of an active teaching program into a busy treatment facility. All first and second year medical students at the University of California, San Diego take part in a series of didactic lectures and small group discussions, while third and fourth year students have the opportunity of joining a treatment team for 4 to 6 weeks. All psychiatric residents spend a minimum of 8 weeks working on the unit, and one fourth year resident is chosen to work with the staff for a year. The interactions between treatment teams and students in various stages of development help maximize enthusiasm and the commitment to the best patient care possible. Vocational training for GDPs--orthodontic module

An orthodontic module for the vocational training scheme has been designed specifically with the needs of trainees in mind. It has the two-fold aim of streaming patients in relation to orthodontic requirement and providing a proper understanding of the very specific nature of interceptive measures so often advocated in relation to orthodontics. Due to its relative geographic isolation, the small area of Northern Ireland is an ideal region for a pilot scheme such as this. Fieldwork Supervision: Roles, Tasks and Responsibilities

Fieldwork constitutes approximately thirty‐five per cent of occupational therapy undergraduate programme of study. It is of concern to occupational therapy educators that the fieldwork portion of the curriculum be planned and implemented to provide educationally sound learning experiences. Whatever the level of training and experience of fieldwork supervisors, implementation of a fieldwork training programme which will provide the necessary levels of learning, is dependent on the supervisor's understanding and use of methods of supervision. These include a knowledge of learning theories, skill in directing, supervising and guiding students in the performance of desired action. Paramedicals' Clinical Accuracy: In 102 Cases Referred to a Provincial Hospital

A random sample of 102 cases referred in 1983 by paramedicals to the Southern Highlands Provincial Hospital, Papua New Guinea, was analysed for agreement with the final hospital diagnosis and with standardized primary care therapies. Health centre diagnosis was judged accurate in 45% of cases; inaccuracies in a further 17% had no projected health consequences. However, resulting serious sequelae could be projected in half of the incorrect diagnoses. Diagnosis of surgical cases was more often accurate (P<0.05) than in medical cases. Treatment which was adequate or standard for the diagnosis made at the health centre had been given there in 80% of cases. However, there were serious diagnostic inaccuracies in 38% of all referral cases; this study suggests a need for problem-based paramedical education in diagnosis, especially in non-surgical problems. Visits by doctors to health centres could reinforce this diagnostic teaching and better evaluate paramedicals' clinical accuracy in the health centre. © 1986, SAGE Publications. All rights reserved.

Education in geriatrics in physical medicine residency training programs

The number of aged persons in the United States is rising steadily. Rehabilitation medicine is becoming increasingly involved in providing the care necessary to keep aging individuals in good health so that the onset of late life disability can be delayed and social independence be maintained for as long as possible. For this reason, education in geriatric rehabilitation must become an essential part of training programs for residents in physical medicine and rehabilitation. The curriculum of these programs must be broad based and must include many of the general aspects of geriatric medicine even though they may relate only peripherally to rehabilitation. One faculty member should be charged with the responsibility of organizing the educational program. He should maintain liaison with faculty members of other departments who have expertise in the problems of the aging. Some of the teaching can be done by lectures and seminars but much of it should be carried out in clinical settings such as in-patient facilities, out-patient departments, home health services and day-care programs. Nursing homes might offer suitable training facilities provided that adequate supervision and teaching are available. Every effort should be made to overcome prejudices that may exist with regard to the care and rehabilitation of a geriatric population.

Vocational training for family practice in Israel

Awareness that the competent family physician in the community is the key person in providing economic and effective health services is increasing. In Israel, as in most Western countries, this need is being met by postgraduate vocational training in family medicine. In Israel, the 4-yr curriculum, after completion of the compulsory internship year, consists of 27 mo in rotating residencies in approved hospital wards, followed by 21 mo in approved teaching practices, under the supervision of family physician tutors. A formal course of academic studies or work in one of the basic medical sciences is required, as are the Board of Family Medicine mandatory examinations. Now it is necessary to reach a consensus on the future scope and content of family medicine as an independent discipline that can provide comprehensive, continuous, accessible, and coordinated health care to all family members in the practice population, both in their homes and in clinics. There is also a need to recognize that academic and clinical teaching and research are as much a part of the family physician's task as is his service commitment, and that adequate facilities must be provided.

Application of humanistic learning theory in an associate degree program for occupational therapy assistants

A humanistic education model was implemented in a 2-year associate degree curriculum for occupational therapy assistants. The goal of this model was to help students to become responsible and successful with their own lives. This prepares students with higher levels of ability to cope effectively with clients in crises. The curriculum presented here supports the framework of The American Occupational Therapy Association role delineation document (Entry-Level Role Delineation for OTRs and COTAs, approved by the Representative Assembly, AOTA, March 1981).
